Pain Diary in a Pediatric Patient ( Child)

If a child has pain then it would be advisable to maintain a pain record before he/she is taken  to the doctor. This will help the doctor in deciding treatment for the  child. It should have record of preferably three bouts of pain. It should contain the following:
 

Date and time of the day

Place : school, home or playground

People present.

Describe the pain;    Colicky, constant, aching, etc.

Where on body                How it lasted            How bad was it ( approximately grade from 1-10)

What did it stop the child from doing - study, play, etc.

Trigger for pain;

Treatment given;

        Whatever treatment is given before seeing the doctor.

Your Observations;

        Your observation of the effect of the treatment or any other details of the child's condition.
